This project consists of a detailed written case summary, formulation, and treatment plan for a hypothetical patient. The project should include any background, history, interviewing and test data/results available on the patient as well as the student's case formulation and treatment plan. Any previous assessment results and report can be used as a starting point for a more detailed formulation and treatment plan.
In this project, specify a theoretical orientation or combination of orientations to develop a conceptualization of the hypothetical case (i.e. Behavioral, Cognitive-Behavioral, Psychodynamic Systems, Client-Centered/experimental). This theoretical orientation should then be used to present a thorough understanding and conceptualization of the patient's presenting problems, symptoms, or issues, history and any available testing data.
A complete, five axes DSM-IV-R diagnosis is also required, followed by a detailed treatment plan with goals and specific recommendations. Also, include some discussion of any potential problems or obstacles to treatment that might exist for the patient.
